About NY State Department of Health NHTD

The NY State Department of Health NHTD waiver program is a home and community-based program that helps New York´s Medicaid-eligible seniors and people with physical disabilities receive comprehensive services they need while they live in a community-based setting, rather than in a nursing home, congregate care setting, or other institution.

The NY State Department of Health NHTD Waiver Program supports individuals who want to remain in their homes and communities while receiving personalized services designed around their goals and needs.

PERSON-CENTERED PLANNING

Individuals with physical disabilities and seniors have the same rights as everyone else:

  • to control their own lives

  • to make informed choices

  • to take reasonable risks

  • to learn and grow from experience

Participant satisfaction is a key measure of success.

The NY State Department of Health NHTD program balances independence with responsibility for health and safety.

PERSON-CENTERED PLANNING

Participants work in cooperation with providers and a case coordinator to develop a Personalized Service Plan.

This process leads to:

  • Greater independence

  • Increased community involvement

  • Personal empowerment

  • Higher quality of life

 

Participant satisfaction is a key measure of success.

HOUSING SUPPORT

Depending on funding availability, participants may qualify for a state-funded housing subsidy to support community living.

Housing is not a waiver service, but financial assistance may be available through the NY State Department of Health NHTD program.
